We are so thankful you have chosen us as your care provider. Please allow 2 hours to complete a new patient registration form and health history questionnaire prior to your appointment. A healthcare provider will perform necessary assessments regarding your visit will have medications prescribed, blood work, or dental x-rays done if needed. 

WHAT CAN I EXPECT:

Please Allow up to two hours to complete a new patient registration form and health history questionnaire, be seen by a healthcare provider, have medications prescribed and have blood work or dental x-rays done, if needed. A welcoming, comfortable, clean, and professional medical environment awaits you! Your privacy, dignity and well being are very important to us.

What Do I Need To Bring:

We cannot submit your application(s) for free medication without Proof of Household Income for everyone living in the home. These include:

  • Face Mask

  • Drivers License or State Issued ID

  • Proof of Residency Bill (Bill or statement)

  • Proof Of Household Income

  • SSI

  • Insurance Card if Insured

WHAT IS PATIENT ASSISTANCE?

St. Martin’s Healthcare assists our patients with obtaining their prescribed medications through a variety of pharmaceutical donation programs. Each pharmaceutical company has its own requirements and forms to be completed and signed. We understand that this process can be overwhelming and will help you apply for and receive the medication(s) you may be qualified to receive.

What do I need for St. Martin's Healthcare to Assist Me in Applying for Free Medications?

We cannot submit your application(s) for free medication without Proof of Household Income for everyone living in the home. These include:

  • All check stubs from the last 30 days

  • Current year Federal Tax form 1040

  • Child support

  • TANF

  • SSI

  • Pension

  • Disability

  • Social Security

  • Unemployment income

How do I obtain Refills from St. Martin's Healthcare?

Once you are approved by the drug company, you will receive a 2-3 month supply of your medication. We will call you when it arrives. If someone else is picking up your medication, you must send a note giving them permission.

When you have a 1 month supply left, it is your responsibility to call the clinic (260) 357-0077 ext 305 to reorder. We cannot guarantee you will be able to get your medication unless you call to reorder.

Yearly re-enrollments for each medication are needed. You must continue your follow-up visits with the clinic’s healthcare provider as required.

COVID-19 APPOINTMENT POLICY

If you have any COVID-19 symptoms SUCH AS cough or shortness of breath, fever, chills, headache, muscle aches, or new loss of taste or smell or been exposed, we will not see you IN the clinic but will conduct a Telehealth visit and or COVID-19 testing. We offer COVID-19 testing for those who are eligible or patients at the clinic. For more information visit our COVID-19 vaccine information page.
