Please understand that we are limited to serving only community members living in DeKalb, Noble, Stueben, and LaGrange County.
AM I ELIGIBLE FOR MEDICAL CARE?
Must reside in DeKalb, Noble, Stueben, or LaGrange County
Must be uninsured
Must be underinsured
Be at or below 200% of the Federal Poverty Guidelines
YOU ARE NOT ELIGIBLE FOR MEDICAL CARE IF:
Employer-sponsored Private Health Insurance/HSA
Medicaid, Medicare, or HIP
Injuries sustained while on the job (workers comp)
Injuries from motor vehicle accidents
VA Benefits
AM I ELIGIBLE FOR DENTAL CARE?
YOU ARE NOT ELIGIBLE FOR DENTAL CARE IF:
Must reside in DeKalb, Noble, Stueben, or LaGrange County
Must be 18 years of age or older
Have no dental insurance or benefits
Be at or below 200% of the Federal Poverty Guidelines
Employer-sponsored Private Health Insurance/HSA
Medicaid, Medicare, or HIP plans offering dental coverage
Injuries sustained while on the job (workers comp)
Injuries from motor vehicle accidents
VA Dental Benefits
